We focus our care on all adults (21+), with special attention to seniors and complex patients.

To ensure the best possible care and medical advice, we would appreciate all patients bring their ID, insurance information, any old medical records, and their current medication list and/or prescription bottles.

We pride ourselves in minimizing wait times and optimizing your experience with check in and payment process. For this reason. we ask all our patients to arrive about 15 minutes prior to the scheduled appointment.

We strive to have a respectful atmosphere in our office for the provider, staff, and of course, for our patients.

For this reason, any patient who arrives more than 15 minutes after their scheduled appointment time will be asked to reschedule their appointment. Any patients who fail to arrive to their appointments to 2 or more appointments will be discharged from our practice.

Our goal is to answer all incoming phone calls, e-mails, and fax to our office in a timely manner. Sometimes due to heavy call volumes, we may not be able to get to your request right away.

We ask to allow 24 business hours for a return call and 48 hours for E-mail response. For any urgent matters, we ask that you call 911. If you have a medical symptom, complaint, or request for consultation on any medical matter, we will help facilitate an appointment to speak with the provider.

Refills must be requested at least 3 business days prior to running out of medications. You may contact our office by phone or e-mail. Refills will not be filled on holidays, weekends, or after business hours.

We extend refills to our patients who are being regularly seen at their recommended appointment time frames (as decided at visits). If you are overdue for an appointment, every attempt will be made to either schedule an appointment as soon as possible or a temporary supply will be sent until patient is seen in office.

We adhere to all Federal and State regulations for controlled substances. For full transparency, at this time, we are currently NOT providing long term management for:

1. Long term chronic pain medication with opiate medications (i.e. Norco, Percocet, etc).

2. Long term insomnia medications with Ambien and similar prescriptions

3. Long term anxiety medications with Benzodiazepines (such as Alprazolam, Lorazepam, Triazolam, etc.)

4. ADHD management

5. Long term testosterone replacement

When indicated, a patient will be referred to an appropriate specialist provider to render long term management of the above conditions and every attempt will be made to help bridge prescriptions until care is established with a specialist provider (not exceeding 3 months for bridging prescriptions).

On certain case by case basis, short term prescriptions may be considered if appropriate to the patient’s disease process based on the provider’s discretion.

If a referral is needed for a patient, an order will be placed with insurance within 48 hours of your visit. Total processing time for referrals may take up 1 week.

Forms such as FMLA or short term disability (or other forms requiring signature) will require an in person visit to detail the need for completion of forms. All submitted paperwork may take up 7-10 days to return to the patient.

Any request for short term disability will, in addition to the above, require patient participation with a specialist provider (if appropriate) and compliance with all testing and imaging needed to help determine the nature of the disability.
